In-Person Registration: To complete in-person registration, download the appropriate form below, print, and fill out. Completed forms may be brought to the Connect Center on the days/times outlined below. Additional forms are available upon request at the Connect Center.
Online Registration: To complete in-person registration, download the appropriate form below, then fill out all required information online or print, fill, and scan. After completing the form, attach and email the file to info@cantonconnectcenter.org.
